1Acceptance of Terms
By accessing or using strengthmotiontherapy.com (the "Site") and any related services (collectively, the "Services"), you agree to be bound by these Terms of Service ("Terms"). If you do not agree to these Terms, please do not use the Site or Services.
2Eligibility
- You must be at least 18 years old to book services or create an account.
- By using the Services, you represent and warrant that you have the legal capacity to enter into a binding agreement and have provided accurate information.
3Description of Services
We provide in-home physical therapy services to clients located within our specified service area. Licensed therapists will come to your residence (home, assisted living, etc.) to deliver care.
The Site's primary functions:
- Display information about therapists and services.
- Allow you to submit booking requests.
- Facilitate communication between you and our staff.
4Account Registration & Security
- To book an appointment, you may need to create an account and provide accurate PII as described in our Privacy Policy.
- You are responsible for maintaining the confidentiality of your account credentials (email and password) and for all activities that occur under your account. Notify us immediately at strengthmotiontherapy@gmail.com if you suspect unauthorized use.
5Booking & Payment Terms
Appointment Requests
- All bookings made through the Site are requests and subject to availability. We will confirm or propose alternative dates/times via email or phone.
- You must provide at least 24–48 hours' notice for scheduling or canceling appointments (see "Cancellation Policy" below).
Fees & Invoices
- Service fees may vary by therapist, location, and type of therapy. Published rates on the Site are subject to change; we will notify you before your session if fees change.
- Insurance Billing: If you choose to bill your insurance, you are responsible for any co-pays, deductibles, or non-covered services as determined by your policy.
- Self-Pay Clients: Payment is due prior to or at the time of each session, unless otherwise arranged. We accept credit/debit cards and checks.
Cancellation & No-Show Policy
- Cancellations made at least 48 hours before a scheduled session are not charged a fee.
- Cancellations within 24–48 hours may incur a partial charge (e.g., 50% of session fee).
- Cancellations less than 24 hours or no-shows will be charged the full session fee.
- We reserve the right to waive fees in extenuating circumstances (e.g., emergencies).
